Chronic Disease Self-Management Program

The Chronic Disease Self-Management Program (CDSMP) is a state-wide “best-practice” for older adults facing chronic disease. The program focuses on empowering older adults to make lifestyle and behavioral changes to improve health.  CAC, partnering with the Indiana State Department of Health and the Indiana Division of Aging, has three specific project goals:

  1. Cultivate the personnel and infrastructure to develop and implement high quality Chronic Disease Self-Management (CDSMP) curriculum across Indiana.

  2. Build upon aging network partnerships to assure program sustainability.

  3. Provide formative and summative evaluation of CDSMP implementation centrally and across sites to assure continuous quality improvement.
